29 CFR  1910.23(c)(7)(i):  Portable Ladders.  The employer must ensure ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic are secured to prevent accidental displacement.  On or about July 18, 2024, in the Wall Production Loading area at 10805 Business Hwy 51 South in Rothschild, Wisconsin, employees were exposed to potential fall hazards 10 feet above surface level while using straight ladders with the tops positioned against prefabricated walls that were hoisted by overhead cranes.  The ladders were not secured to prevent accidental movement from crane operation.

29 CFR  1910.28(b)(1)(i):  Unprotected sides and edges.  Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:  guardrail systems, safety net systems, or personal fall protection systems.  On or about July 18, 2024, in the Wall Production Loading area at 10805 Business Hwy 51 South in Rothschild, Wisconsin, employees were exposed to a fall hazard greater than four feet above a lower surface when climbing on top of interior wall packs to secure the packs to the shipping trailers.

29 CFR  1910.135(a)(1):  The employer shall ensure that each affected employee wears a protective helmet when working in areas where there is a potential for injury to the head from falling objects.   On or about July 18, 2024, in the Wall Production Loading area at 10805 Business Hwy 51 South in Rothschild, Wisconsin, employees were exposed to struck-by hazards operating overhead crane hoists when rigging prefabricated walls and loading the walls onto trailers without using head protection.

29 CFR  1904.39(a)(2):  Reporting Fatality, Injury and Illness Information to the Government.  Basic Requirement.  Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about July 18, 2024, at 10805 Business Hwy 51 South in Rothschild, Wisconsin, the employer had failed to report a hospitalization event within 24 hours of the known admission date of July 18, 2024.
