29 CFR  1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a. STAR Turbine, Inc. - Marion, Ohio: On or about July 25, 2024, the employer did not ensure that employees who setup and maintain the Toshiba vertical turret lathe (model TUE-150; s/n 440589) were trained in the skills required for the safe application, usage, and removal of energy controls.  Employees were not trained to isolate and render energy controls safe prior to performing setup tasks.   b. STAR Turbine, Inc. - Marion, Ohio: On or about August 1, 2024, the employer did not ensure that employees who setup and maintain the #1 Century vertical turret lathe (s/n 2706) were trained in the skills required for the safe application, usage, and removal of energy controls. Employees were not trained to isolate and render energy controls safe prior to performing setup tasks.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a. STAR Turbine, Inc. - Marion, Ohio:  On or about July 25, 2024, employees were exposed to rotating parts and pinch point hazards in that the Toshiba vertical turret lathe (model TUE-150; s/n 440589) was not deenergized prior to set-up tasks.  The employer did not ensure that energy control application steps were implemented to control hazardous electrical energy sources prior to the servicing tasks - the employer did not: -Ensure employees had the method or means to control hazardous energy. -Ensure that equipment was turned off or shut down. -Physically operate energy isolating devices to control energy sources and affix lockout device(s). -Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and  rendered safe. -Verify that isolation and deenergization of the machine or equipment had been accomplished.  b. 29 CFR  1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used.   a. STAR Turbine, Inc. - Marion, Ohio:  On or about August 1, 2024, the employer used a twist-lock carabiner as a lifting attachment to lift packing ring parts in Bay 2. Employees that used the makeshift lifting attachment were exposed to struck-by and crushing injuries in that the attachment was not designed and manufactured as a lifting device.   No additional abatement information is needed for this item.

29 CFR  1910.184(f)(1): Wire-rope slings did not  have permanently affixed and legible identification markings as prescribed by the manufacturer  that indicated the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one:   a. STAR Turbine, Inc. - Marion, Ohio: On or about August 1, 2024, the employer did not ensure that the two wire rope slings, used a Bay 2 to lift packing ring parts was equipped with permanently affixed identification to include information such as, but not limited to, the sling's safe working load for the type of hitch used, the angle upon which it is based, and number of legs.   No additional abatement information is needed for this item.

STAR Turbine, Inc. - Marion, Ohio: On or about August 1, 2024, employees who operated the Toshiba vertical turret lathe (model TUE-150; s/n 440589) were exposed to injuries associated with the machine's rotating parts and pinch points.  The employer did not ensure the machine was adequately guarded to prevent employees from accessing the hazard area during operation.   b. STAR Turbine, Inc. - Marion, Ohio: On or about August 1, 2024, employees who operated the #1 Century vertical turret lathe (s/n 2706) were exposed to injuries associated with the machine's rotating parts, pinch points, and flying debris.
