29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface: On or about August 2, 2024, employees engaged in retail optometry operations at Eyeglass World located at 5555 O Street, Lincoln, Nebraska were exposed to shock and electrocution hazards in that the light switch faceplate in the General Manager's office was broken and did not completely cover the live internal switch connections.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On or about August 2, 2024, employees performing laboratory activities in the laboratory of Eyeglass World located at 5555 O Street, Lincoln, NE were exposed to electrical shock hazards in that the employer failed to identify the frayed and damaged outer insulation on the Coburn CS-7 power cord and remove it from service before use.
