29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: (a) On or about July 30, 2024, and at times prior, at 7675 Jacks Way in Horace, North Dakota, two employees installing wall sheathing from a platform attached to a JCB Model #532 Serial #SLP53200XE0774563 Telehandler were at risk of tipping. The platform was not designed by a qualified person�to serve as a scaffold or as a working platform for employees; rather, it was intended to raise and lower material. There was a tipping hazard since the platform extended more than 18 inches past the wheelbase on each side.
