29 CFR 1926.251(a)(4): Lifting accessories for material handling were not marked to indicate the safe working loads and were not proof tested prior to use to 125 percent of their rated load(s): a) 311 Crescent Blvd., Brooklawn, NJ - The employer allowed workers to hoist metal roof sheathing aloft using an approximate 20 foot by 8 inch wide steel C-Channel (lifting accessory), resting atop the forks of a Genie Rough Terrain Forklift Truck, with no markings to indicate the safe working loads nor being proof tested prior to its use, thus exposing workers to struck-by and caught-in-between hazards. This condition was noted on or about July 30, 2024. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.602(c)(1)(ii): Additions affecting the safe operation of the equipment were made without the manufacturer's written approval: a) 311 Crescent Blvd., Brooklawn, NJ - An employee was permitted to use an addition to the forks, metal C-Channel as a lifting beam, to lift a bundle of metal roof sheathing without the manufacturers written prior approval. The forklift with C-Channel was not labeled to identify the attachment or combined weight. Thus exposing employees to struck-by and caught-in-between hazards. This condition was noted on or about July 30, 2024. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
