29 CFR 1910.147(d)(6):Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished: On or about August 23, 2024, while trouble-shooting the Weinig Opticut Dimter Line 450 Quantum II, an employee suffered an amputation of his index finger. When the employer failed to ensure procedures were adequate to ensure that the stored energy (continual movement of the belt and saw) came to a stop prior to any maintenance activities.
