29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) Warehouse - On or about July 25, 2024, an employee sustained a left index fingertip amputation while cleaning the Line 1 Townsend Flex-A-Matic NL SMARTLinker stuffing machine. The employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. b.) Warehouse - On July 31, 2024, an employee performed the service and maintenance task of disassembling the Line 1 Townsend Flex-A-Matic NL SMARTLinker stuffing machine. The employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.

An employee reached into the gear box while the machine was running, sustaining a left index fingertip amputation.
