On or about August 2, 2024, seven employees were walking/working on a roof greater than 42' feet above lower level and were not protected from falling.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Location - On roof peak at the job site. On or about August 2, 2024, an employee was exposed to injury while working/walking on a commercial roof where the fall protection system used "waist belt" and not full body harness .
