29 CFR  1910.22(a)(2):(2) The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.  a.  On or about August 5, 2024, the floors in the kitchen were not kept in a clean and dry condition, which exposed employees to slip, trip, and fall hazards from the regular accumulation of water.  b.  On or about August 5, 2024, the floors in the refrigerated/frozen storage sections of the restaurant were not kept in a clean and dry condition, which exposed employees to slip, trip, and fall hazards from the accumulation of ice along the seal of the malfunctioning freezer door.

29 CFR  1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access.  a.  On or about August 5, 2024, the corridor serving as an exit access in the rear of the workplace was partially blocked with pallets of bread, which narrowed the usable width of the exit access to approximately 23".

29 CFR  1910.157(e)(3):The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check.   a.  On or about August 5, 2024, the Class K portable fire extinguisher in the restaurant kitchen last underwent an annual maintenance check in March of 2023, placing it approximately five months beyond its required re-check date.
