29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Formulation area - On or about 07/15/2024, and at times prior, the employer exposed employees to trip hazards in that, the employer failed to ensure that the walking/working surfaces were maintained in a clean and orderly condition while employees were engaged in activities involved preparing chemicals such as, but not limited to, Acticide MV (corrosive) to go on pallets.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  a) Formulation area - On or about 07/15/2024, and at times prior, the employer exposed employees to chemicals hazards in that, the employer failed to ensure training was provided to employees for exposure to hazardous chemicals to protect employees with appropriate work practices, emergency procedures and PPE to use while working with hazardous chemicals, such as, but not limited to, Acticide MV( corrosive) to go on pallets.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) Throughout the facility - On or about 07/19/2024, the employer failed to report to OSHA within 24 hours a work-related in-patient hospitalization of an employee when the employer reported employee's hospitalization to OSHA on 7/26/2024. Employee was hospitalized on 7/19/2024 and released from the hospital on 7/23/2024.
