29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Formulation area - On or about 07/15/2024, and at times prior, the employer exposed employees to trip hazards in that, the employer failed to ensure that the walking/working surfaces were maintained in a clean and orderly condition while employees were engaged in activities involved preparing chemicals such as, but not limited to, Acticide MV (corrosive) to go on pallets.

Employee was hospitalized on 7/19/2024 and released from the hospital on 7/23/2024.
