29 CFR  1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  a) Pipe Shop, Mechanical Equipment Room #5 (Bldg. 100): On or about July 22nd, 2024 the designated emergency exit doors that opened into the exit access were not equipped with exit signage.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1651105, citation no. 2, item no. 1, and was affirmed as a final order on May 23rd, 2023 with respect to a workplace at Vancouver, WA.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(b)(2), which was contained in OSHA inspection number 1650384, citation no. 2, item no. 1, and was affirmed as a final order on May 23rd, 2023 with respect to a workplace at Vancouver, WA. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Energy Plant Engineering Shop, Break Room (Bldg. 100): On or about July 22nd, 2024 a Tripplite relocatable power tap (RPT) surge protector (Model No. TLP606 Series #AG-0015) was used to provide power to a Keurig Single Serve coffee maker and a Mr. Coffee coffee maker, exposing employees to electrical hazards.  b) Pipe Shop (Bldg. 100): On or about July 22nd, 2024 three relocatable power tap (RPT) surge protectors were daisy chained and used to supply power to computers, monitors, charging docking stations, and printers, exposing employees to electrical hazards.  The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1706199 citation no. 1, item no. 1, and was affirmed as a final order on December 12th, 2023 with respect to a workplace at Spokane, WA.

29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  a) Pharmacy Out-Patient (Bldg. 100): On or about July 24nd, 2024, a control panel for a Omnicell ROBOT-Rx Central Pharmacy Robotic System was positioned and bolted in front of the facility's electrical panel, delaying emergency access, interfering with proper equipment operations, and compromising equipment inspections.   The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(ii), which was contained in OSHA inspection number 1607692, citation no. 1, item no. 4 and was affirmed as a final order on September 1st, 2022 with respect to a workplace at Walla Walla, WA.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure:  a) Pharmacy Out-Patient (Bldg. 100): On or about July 24nd, 2024, in lieu of permanent wiring, an (12 gauge) orange-colored extension cord was used in order to provide power to a Thermo Scientific Refrigerator. Employees who use the refrigerator to storge sampling media were not protected from minor shock and/or  potential electric hazards if the extension cord became compromised   The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1687933, citation no. 1, item no. 2, and was affirmed as a final order on September 20th, 2023 with respect to a workplace at Yakima Valley, WA.
