29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: a) Prosthetics Laboratory: On or about July 22nd, 2024, a JET 20" Bench Drill Press (Serial No. 11040789) was not equipped with an adequate guard, exposing employees to hazards created by rotating parts, reciprocating motion and significant amounts of heat that can cause skin burns, flying sparks, and fire hazards. b) Prosthetics Laboratory: On or about July 22nd, 2024, a CLAES Sewing Machine (Masch.-Nr. 337244) was not equipped with an adequate guard, exposing employees to hazards created by the sewing needle's piercing motion part that can cause entanglement, eye and/or finger injuries. c) Prosthetics Laboratory: On or about July 22nd, 2024, a MicroLux� "True Inch" 7x16 Variable Speed Lathe was not equipped with an adequate guard, exposing employees to hazards created by rotating parts, reciprocating motion, flying debris, pinch points, and possible machine malfunction.

29 CFR 1910.215(d)(3): The contact surface of wheel on grinding machine was not flat and free or foreign matter: a) Prosthetics Laboratory: On or about July 22nd, 2024, the wheel (right wheel) on the Baldor Pedestal Grinder (Identifying No. 3415 5000) was not flat and contained foreign material on its contact surface, exposing employees to high levels of vibration, injuries to the operator's hand, fire hazards, and potential contact with from projectiles or flying debris.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure: a) Research Laboratories (Bldg. 101, Room 728): On or about July 24nd, 2024, a black-colored extension cord was used as a substitute for fixed wiring in order to provide power to a MovinCool Portable AC-1, exposing employees to electrical hazards. The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1687933, citation no. 1, item no. 2, and was affirmed as a final order on September 20th, 2023 with respect to a workplace at Yakima Valley, WA. The U.S. Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1650387, citation no. 1, item no. 3, and was affirmed as a final order on May 23rd, 2023 with respect to a workplace at Vancouver, WA.
