29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used: a) Yard at 133 Rocky Drive, Carlton, GA: On or about August 6, 2024, employees operating a bridge crane to lift granite slabs were exposed to a struck-by and crush hazard, in that the employer did not remove a polyester sling, S/N 17757, from service that had visible damage such as but not limited to cuts, frays, exposed threads, and an illegible tag.
