Intersection of South Whitney Street and West Boulevard, Hartford, CT: On or about June 12, 2024, employees flagging traffic at a roadway excavation for utility installation without having adequate training in the recognition of the appropriate set up and placement of traffic control signage, devices, proper flagger tools and methods, and flagger stations, which exposed the employees to vehicle traffic struck-by hazards.

Intersection of South Whitney Street and West Boulevard, Hartford, CT: On or about June 12, 2024, the employer failed to ensure that an employee signaling and controlling the flow of traffic at West Boulevard and South Whitney Street did not use a stop/slow paddle, flag or automated flagger assistance device as per Part 6 Section 6E.07 of the MUTCD (2009 Edition).
