29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program.   Naval Air Station North Island, M/V USS George Washington (CVN-73), Pier K: On or about August 6, 2024, and at times prior thereto, the employees conducted deck removal tasks utilized heavy machinery including needle gun(s) and jackhammer(s) without noise level measurements being conducted to monitor sound level(s) of the work areas. This condition potentially exposed employees to, tinnitus, temporary and/or permanent hearing loss.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer.  Naval Air Station North Island, M/V USS Gorge Washington, Pier K: On or about August 6, 2024, and at times prior thereto, the employees donned 3M Model 6200 half-face air purifying respirator with particulate cartridges while conducting deck removal tasks with needle gun and jackhammer in the designated spaces. The half-face respirator's components were not cleaned and disinfected appropriately after each use which potentially exposed employees to dermatitis and/or inhalation hazards.

29 CFR  1910.134(h)(2)(i):Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve.  Naval Air Station North Island, M/V USS Gorge Washington, Pier K: On or about August 6, 2024, and at times prior thereto, the employees donned 3M Model 6200 half-face air purifying respirator with particulate cartridges while conducting deck removal tasks with needle gun and jackhammer in the designated spaces.
