Location: 151 MA-130 Sandwich MA On July 30th 2024, employees were exposed to falls from heights over thirty feet to the ground while working from a ladder scaffold system on a commercial roof where a competent person qualified in scaffold erection was not present to supervise or direct the scaffold erection.
