29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about August 7, 2025, at 3400 South Dakota, NE, Washington, D.C. employer exposed employee to a fall hazard of approximately 20 feet while working on a roof to install shingles without the use of fall protection.    No abatement certification or documentation required for this item.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid  (a) On or about August 7, 2025, at 3400 South Dakota, NE, Washington, D.C. the employer exposed employee to a fall hazard while using a portable ladder which was not able to be extended three feet above the roof landing.    No abatement certification or documentation required for this item.
