On or about On or about April 1, 2024, to July 28, 2024, at an establishment located at 1173 West Johnson Street in Fond du Lac, Wisconsin; employees were exposed to slipping and tripping hazards when the floor drain in the area of the battery filling station backed up with sewage from the nearby toilet facility.

On or about June 16, 2024, to August 8, 2024, at an establishment located at 1173 West Johnson Street in Fond du Lac, Wisconsin; employees filling batteries with diluted battery acid were exposed to corrosive splashes to the eyes and face while not wearing a protective face shield.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. On or about June 16, 2024, to August 8, 2024, at an establishment located at 1173 West Johnson Street in Fond du Lac, Wisconsin; employees filling batteries with diluted battery acid were exposed to corrosive splashes to the eyes where there were no suitable facilities for flushing of the eyes for immediate emergency use. The eyewash station located adjacent to the battery fill area did not have enough water pressure to project the water high enough above the outlets for use.
