29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): a) On August 07, 2024, an employee was exposed to cave-ins while sandblasting a natural gas pipeline in an unprotected excavation with Type B soil, measuring approximately 9-feet-deep. The bottom 6-feet of the west wall was sloped at an angle of approximately ?:1 before sloping to an angle of 1:1. The top of the excavation measured approximately 36-feet wide. Blue Sky Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.652(a)(1)), which was contained in OSHA inspection number 1611723, citation number 1, item number 2b and was affirmed as a final order on 01/03/2023, with respect to a workplace located at the SE Corner of Fairview and Eagle Rd., Meridian, ID 83642. Note - Abatement certification and documentation are required for this item.
