29 CFR 1910.28(b)(13)(ii): (ii) When work is performed at least 6 feet (1.6 m) but less than 15 feet (4.6 m) from the roof edge, the employer must ensure each employee is protected from falling by using a guardrail system, safety net system, travel restraint system, or personal fall arrest system.  The employer may use a designated area when performing work that is both infrequent and temporary.   On or about August 8, 2024, at location 202 South Johnson Street, Berlin, Wisconsin: The employer did not ensure employee safety from fall hazards during the servicing of rooftop air conditioning units and filter replacement. Employees were working close to an unprotected edge, facing fall hazards of approximately 13 feet without the protection of a personal fall arrest system or guardrails.

29 CFR  1910.30(a)(1):(a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about August 8, 2024, at location 202 South Johnson Street, Berlin, Wisconsin: The employer did not provide fall hazard training to employees exposed to a 13-foot fall while working on the building's roof.

Employees shall be trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertain to their respective job assignments.   On or about August 8, 2024, at location 202 South Johnson Street, Berlin, Wisconsin: On or about August 8, 2024, at 202 South Johnson Street, Berlin, Wisconsin, the employer failed to provide necessary training to employees exposed to electrical hazards. The employer did not deliver electrical safety training regarding safe work practices and procedures for employees performing maintenance and testing of electrical equipment to verify de-energization before working on the electrical system.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed.   On or about August 8, 2024, at 202 South Johnson Street, Berlin, Wisconsin, the employer failed to ensure that every employee exposed to electric arc hazards was provided with protective clothing and equipment.
