On or about 8/7/2024, at the excavation located at the north side of the jobsite at 851 Gulf Shore Blvd N, Naples, FL 34102, an employee was exposed to struck by and cave-in hazards while working inside an excavation 7-foot deep with spoil pile placed at the edge of the excavation.

On or about 8/7/2024, at the excavation located at the south side of the jobsite at 851 Gulf Shore Blvd N, Naples, FL, 34102, employees were exposed to a cave-in hazard while working inside an excavation 6.9-foot deep that lacked an adequate protective system. b. On or about 8/7/2024, at the excavation located at the north side of the jobsite at 851 Gulf Shore Blvd N, Naples, FL, 34102, an employee was exposed to a cave-in hazard while working inside an excavation 7-foot deep that lacked an adequate protective system.
