1910.145(c)(2)(i) Caution signs were not used to warn against potential hazards or to caution against unsafe practices: 1886 Upper Maple Street Dayville, Connecticut, Buffer Area. On, or about August 3, 2024 an employee fell approximately 23 feet from the seventh level to ground, fracturing their heel and back while performing cleaning tasks on the rack system. There were limited administrative and engineering controls in place for employees performing this task at the time of the incident.
