On or about August 7, 2024, in the Final Paint (spray booth), the employer did not ensure to cover the flooring with noncombustible material. The flooring for spray booth #1 was covered with (combustible) carton material that do not facilitate safe cleaning and removal of residues.

On or about August 7 2024, in the Final Paint (spray booth), the employer did not ensure a clear space of 3 feet was maintained from storage of combustible materials. Storage included but not limited to combustible carton boxes and, plastic containers.
