29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility: On or about August 3, 2024, The Bar Turner #3 machine was not locked/tagged out before an employee bypassed the guard (cover) to the #1 polisher to the polisher section of the Bar Turnner#3 machine and was adjusting the guide rest when another employee turned on the machine. This caused the machine to activate and the employee working under the #1 polisher guard to suffer an injury. IN THE ALTERNATIVE 29 CFR 1910.147(d)(2): The machine or equipment is not turned off or shut down using the procedures established for the machine or equipment: (a) Facility: On or about August 3, 2024, the Bar Turner #3 machine was not shut down using the established procedures before an employee lifted the guard (cover) at the #1 polisher section of the Bar Turner #3 machine.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) Facility, Bar Turning 3 Machine, (BT3 machine): On or about August 3, 2024, the employer did not provide training to ensure that employee understood the purpose and function of the employer's energy control program. Also, the employees did not have the knowledge and skills needed for the safe application, usage, and removal of the energy controls needed by the employees.
