29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) On or about August 12, 2024, located at, 8300 SW 8th Street, Miami FL. 33144, the employer did not ensure the scaffold was fully planked, exposing employees to a 7-feet 11-inch fall hazard while employees applying stucco cement to the exterior of a 4-story building.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairway's/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) On or about August 12, 2024, located at, 8300 SW 8th Street Miami FL 33144, the employer did not provide employees with a portable ladder, hook on ladder, attachable ladder stair tower, stairway type ladder, or a ramp to access the scaffold, thereby exposing employees to a 7-feet 11-inch fall hazard while observed climbing the sides of the scaffolding and cross braces to access the upper levels of the 40-foot scaffolding they were using to apply stucco cement to the sides of the exterior building.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below.  a) On or about August 12, 2024, located at, 8300 SW 8th Street Miami FL 33144, the employer did not ensure workers working below the scaffolding were protected from being struck by falling tools by installing toeboards to the scaffolding while employees using hand tools to apply stucco to the sides of the building 30-feet above while other workers were working below the scaffolding.
