Location: Worksite  On or about August 9, 2024, an employee was not protected for falling 50 feet to the pavement below while removing scrim netting from a pipe staging scaffold system.  Actual Incident. On August 9, 2024, at approximately 12:41 pm, an employee fell from the top level of a pipe staging scaffold system to the pavement below and sustained fatal injuries
