29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) 1130 W Elizabeth Ave, LINDEN, NJ 07036, Laser Room; The SHANGHAI FEYIU packing (kitting) machine (Model: 420C-7 Serial# 2016076) features an nip point created by a hydraulic sheer/blade(s) in which plastic sleeve is sealed and cut into parts bags, for which specific procedures were not developed and utilized to prevent injury from unintended activation of the operating cycle. Employee(s) such as operator(s) who unjam and/or adjust plastic sleeve / bag(s) were exposed to crushing and cut hazards from having hand / finger(s) caught within the point of operation On or about 8/08/2024, a leased employee reached into the unguarded blade area while the machine was on and cycled, cutting the tip of a finger on their right hand. Condition(s) were observed on or about 8/12/2024. Research and Mfg Corp. of America was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.147(c)(1)], which was contained in OSHA inspection number 1409792, citation number 1, item number 2 and was affirmed as a final order on 12/05/2019), with respect to a workplace located at 1130 W Elizabeth Ave, Linden, NJ 07036.
