29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. (a) 347 Pratt St, Longmont, CO 80501: On August 12, 2024, and at times prior, and at times prior, employees were exposed to falls of approximately 20 feet to the ground below while conducting roofing activities on an existing residential home. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 3584 141st Drive, Thornton, CO, 80602: On or about August 12, 2024, and at times prior, employees were exposed to falls of approximately 20 feet to the ground below while accessing/egressing upper levels of the existing residential home. The ladders were not secured at their top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladders.
