29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: (a) Vicinity of pipefitter tool room: On or about 8/12/2024, and at times prior, the employer exposed employees to fire and electrical hazards, in that two flexible cords were plugged together to run power from a panel box in the electrical tool room to a power tap providing power to a microwave in the pipefitter toolroom, which was not in accordance with instructions in the listing, labeling, or certification.

29 CFR 1926.403(h): Each service, feeder, and branch circuit was not legibly marked at its disconnecting means or overcurrent device to indicate its purpose, nor located and arranged so that the purpose was evident: Azco Electrical Tool Connex: On or about August 12, 2024, and at times prior, the employer exposed employees to electrical shock hazards, in that circuit breakers in a live panel box were not labeled to indicate their purpose while employees used the box to distribute power to lights, appliances, and tools.
