29 CFR 1910.37(a)(3): Exit routes were not kept free and unobstructed. Equipment was placed within the exit route. Stairs or a ramp were not provided where the exit route was not substantially level:   a) 1000 CenterPoint Blvd., New Castle, DE 19720 (Roof): On or about August 15, 2024, Exit routes were obstructed by pipe chases. Maintenance employees had to travel under and over pipe chases that were located in the path of travel to the exit. There was no clear path of travel to the exit.

29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process.   a) 1000 CenterPoint Blvd., New Castle, DE 19720 (Roof Covering Engine Rooms): On or about August 15, 2024, the employer's PHA did not address the hazard of employees reentering the facility during a release event. This hazardous condition exposed employees to severe injury due to the potential of fire and toxic chemical overexposure hazards posed by anhydrous ammonia release.   b) 1000 CenterPoint Blvd., New Castle, DE 19720 (Engine Room Egress Door): On or about August 15, 2024, the employer's PHA did not address the hazard of trapping workers inside the machinery room during an ammonia release when the machinery room door did not open outwards and did not contain panic hardware nor swing in the direction of egress when employees exit from the engine room in the event of an emergency. This hazardous condition exposed employees to severe injury due to the potential of fire and toxic chemical overexposure hazards posed by anhydrous ammonia release.    c) 1000 CenterPoint Blvd., New Castle, DE 19720 (Rooftop-Relief Header/diffuser): On?or?about?August?15,?2024,?the employer's PHA did not address the hazard of rooftop diffuser vents when the vents discharged downward toward the workers instead (i.e., upward) and away from workers on the roof. This hazardous condition exposed employees accessing/egressing from the roof using the fixed ladder system to severe injury due to the potential of fire and toxic chemical overexposure hazards during an anhydrous ammonia relief discharge event.   d) 1000 CenterPoint Blvd., New Castle, DE 19720 (Compressor Thermosyphon Oil Cooler/Shell and Tube Heat Exchanger) On or about August 15, 2024, the employer's PHA did not address the hazard of liquid from the oil cooler relieving into the vapor relief from the compressor, which is only designed for ammonia vapor, in that employees who work in the machinery room were exposed to relief devices which may fail to operate as designed which could result in toxic chemical overexposure and fire hazards resulting in injuries or death.

This hazardous condition exposed employees accessing/egressing from the roof using the fixed ladder system to severe injury due to the potential of fire and toxic chemical overexposure hazards during an anhydrous ammonia relief discharge event.

29 CFR 1910.28(b)(13)(iii)(A): When work is performed 15 feet (4.6 m) or more from the roof edge, the employer did not protect each employee from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system or a designated area. The employer is not required to provide any fall protection, provided the work is both infrequent and temporary.    a) 1000 CenterPoint Blvd, New Castle DE 19720 (roof edge): On or about August 15, 2024, and times prior, maintenance employees inspect, maintain, and repair refrigeration and cooling equipment on the roof, without utilizing a fall protection system, exposing them to an approximate fall of 21 feet to ground.    The Lineage Logistics, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.28(b)(13)(iii)(A), which was contained in OSHA inspection number 1622709, citation number 1, item number 1 and was affirmed as a final order on 3/30/2023, with respect to a workplace located at 7132 Ruppsville Road, Allentown, PA 18106.

29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment:  1000 CenterPoint Blvd., New Castle, DE 19720 (Anhydrous Ammonia Refrigeration Process): On or about August 15, 2024, the employer did not establish and implement procedures to maintain the on-going integrity of process equipment, when the written procedures were not developed for: a) Ammonia vessels b) Piping systems (including piping component such as valves) c) Relief and vent systems d) Emergency shutdown systems e) Controls (monitoring devices, sensors, alarms and interlocks) f) Pumps  Failure to inspect, test and maintain process ammonia equipment can lead to loss of containment and exposure to toxic vapors and fire hazards.

This condition exposed employees to inhalation hazards.
