A steel storage rack cross beam in aisle 45 located at 22614 West 167th Street, Olathe, Kansas entered into the forklift operator's compartment pinning and crushing the forklift operator against the forklift resulting in a fatal injury.

The employer failed to train reach (stand-up) forklift operators to recognize and avoid the workplace specific hazard of storage rack cross members entering the standup forklift operator's compartment.
