29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof: On or about August 3, 2024, the employer did not ensure that employees tasked with troubleshooting of the Doosan Puma VAW 800 Wheel Turning Machine #3811 (on Line 38) were removed from the hazardous area prior to operating the unchuck button on the control box. An employee received an amputation injury, while able to actuate the chuck (from the control box), while having have hands inside of the equipment.
