29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the worksite located at 200 Main Street Kansas City Missouri: Employees were exposed to electrical shock hazards. A grinder was plugged in and used to grind concrete with an extension cord with exposed conductors.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section: At the worksite located at 200 Main Street Kansas City, MO: An employee was exposed to chemical and respiratory hazards in the form of respirable crystalline silica. The employee was using a handheld grinder on dry concrete without use of wet methods or dust collection methods.
