LOCATION: 242 East Berkeley St. On or about August 13th ,2024: The employer exposed an employee to electrocution hazards and related injuries when that employee was permitted to use a pneumatic hammer to remove concrete from the duct bank controlled by Eversource and without Eversource's knowledge or permission.

LOCATION: 242 East Berkeley St. On or about August 13th ,2024: The employer exposed their employees to electrocution hazards and related injuries when an employee was allowed to enter the excavtion and work with a pneumatic hammer to remove concrete encasing a 13.8kV transmission line without an inspection conducted by a competent person that would have determined and alerted him to the dangers.
