29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   On or about August 13, 2024, the employer failed to ensure that employees were trained to recognize hazardous working conditions at a residential roofing job located at 33 Orrlawn Drive, Tallmadge OH 44278 .  Employees were working at this job with no personal protective equipment being worn and were exposed to hazards, such as but not limited to, being struck by and fall from heights.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    On or about August 13, 2024, the employer failed to ensure that employees were protected from a fall from heights of approximately 8 - 12  feet at various levels while performing roofing activities on a residential home located at 33 Orrlawn Drive, Tallmadge OH 44278  Employees working at the site did not have on a personal fall protection system nor were there any other means of fall protection in place.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 13, 2024, the employer failed to ensure that employees were protected from a fall from height of approximately 12 feet while performing roofing activities on a residential home located at Residential Home located at 33 Orrlawn Drive., Tallmadge OH 44278.  The employees were using various extension ladders and had the top of several of the ladders was placed at the roofs leading edge/landing area and were not extended at least 3 rungs above this area.
