One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about August 13th, 2024, at the site address of 600 Lowell St Elyria, Ohio 44035 EM Service Inc., employees operating the Monarch Lathe are exposed to injuries associated with the machine's rotating parts (potential pinch points, flying debris) in that the machine was not adequately guarded to prevent employees from accessing the hazard area during operations.
