On or about August 15, 2024, employees in the warehouse were exposed to tripping hazards in aisles 108-109 from pitted sections of the concrete floor with inlaid remnants of rebar anchors for removed metal shelving.

29 CFR 1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a. On or about August 15, 2024, the employer's emergency eye wash station was blocked by a pallet of plastic wrap, which prevented employees from accessing the sink to flush their eyes in the event of exposure to corrosive materials in the powered industrial vehicle battery changing area.

29 CFR 1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On or about August 15, 2024, the employer failed to ensure forklift operators were re-evaluated at least every three years to ensure safe operations of the equipment. The employer last provided training to employees in November of 2018.
