On or about August 13, 2024, the employer did not ensure the lockout device indicated the identity of the employee applying the devices. Employee applied lock to the power disconnect of 480-volt, MCP-SML02 without indicating the identity of the employee applying the device.

Safety instruction signs shall be used where there is a need for general instructions and suggestions relative to safety measures. a) On or about August 13, 2024, and at times prior thereto; for failing to ensure employees were sufficiently instructed as to next steps to take if a jam cannot be broken up through use of the jam pole or other jam breaking tactics that can be taken without accessing the conveyor.
