29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103) a) Jobsite located at 1815 John F Kennedy Blvd, Philadelphia PA - On or about August 16, 2024 the employer did not ensure that employee required to wear respirators, did not have facial hair between the sealing surface of the respirator facepiece and the face, thereby exposing employees to respiratory hazards.

29 CFR 1926.300(b)(4)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) Jobsite located at 1815 John F Kennedy Blvd, Philadelphia PA - On or about August 16, 2024, the employer failed to ensure that employees operating a handheld grinder while cutting granite and cement mortar was used with the proper guards in place, exposing employees to flying debris, amputations, and lacerations.
