29 CFR  1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired.  On, or about, August 16, 2024, in the following locations, floor holes for the drainage system were not properly covered due to damaged and/or missing covers:  A.	 Near Multi-Set Machine:  Two sections of the metal trench covers were covered with plywood because they were bent and either did not stay in the L-channel guide or created a tripping hazard. B.	Decommissioned Napping Machine:  A metal trench cover had fallen into the floor hole and pieces of cloth had been tied to the machine frame to act as an awareness barrier.   C.	Reconing Area:  Metal trench covers were missing from two sections of the floor holes.  Empty barrels and a tote had been placed over the trench to try to prevent anyone from stepping into it.

29 CFR  1910.303(g)(2)(i):  Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Carbonizer:  On, or about, August 16, 2024, the cover was missing on two pressure switches mounted on the side of the oven enclosure, exposing electrical connections to accidental contact.

The facility was classified under NAICS 313210 and had 41 employees.
