On or about August 16, 2024, at the above addressed jobsite, employees performing residential roofing activities accessed and egressed the roof from a ladder jack scaffold, seventeen (17) feet four (4) inches above ground, were not provided with an access ladder, there by exposing the employees to fall hazards.
