Employees performing this work were exposed to struck-by, caught-in and electrocution hazards when safe practices for maintenance and servicing of equipment, including electrical power de-energization (lockout) were not established.

The employer failed to provide a means for employees to control or lock a main electrical switch in the off position to deenergize electrical lines prior to the installation of a 5 hp, Tech Top Industrial Electric Motor.  An employee suffered fatal injuries when contact was made with the electricity.
