1910.147(c)(7)(i)(B) Each affected employee shall be instructed in the purpose and use of the energy control procedure. On or about August 8, 2024, the employer failed to ensure that the Minster Mechanical Power Press (Model 3, Herion 2493020, Serial #35/38) operators were trained as affected employees. The operators were not authorized employees and were incorrectly clearing jams on this machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about August 8, 2024, the employer failed to follow established procedures for the control of hazardous energy in accordance with 1910.147(d)(1) through 1910.147(d)(6) as follows: a) Prepare for shutdown b) machine shutdown c) Machine isolation d) Lockout/tagout device application e) relieve and/or render safe stored energy f) verification of isolation Employees are clearing jams regularly when the machine is in operation and the machine is not being locked and tagged out prior to clearing a jam.
