29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position: On or about August 17, 2024, and at times prior thereto, near the Shaver Area where employees were exposed to caught-in and electrical hazards, when operating Shaver #1 with an energy isolating switch that was broken. The switch was unable to be locked out with an energy isolating device.
