29 CFR 1926.451(b)(2)(ii): Where scaffolds must be used in areas that the employer can demonstrate are so narrow that platforms and walkways cannot be at least 18 inches (46 cm) wide, such platforms and walkways were not as wide as feasible.  On or around August 13, 2024, workers carried out masonry tasks while walking and working on scaffolding platforms and walkways less than 18 inches wide.

29 CFR  1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below.  On or about August 13, 2024, employees performed masonry outside a building utilizing scaffolding without toe boards, exposing the workers below to falling debris and materials.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or around August 13, 2024, employees worked on a scaffold under the supervision of someone who had not received proper training from a qualified individual. This supervisor was not equipped to recognize the hazards associated with the type of scaffold being used, nor did they understand the procedures for controlling or minimizing those hazards.

29 CFR  1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the impalement hazard.  On or about August 13, 2024, employees performed masonry tasks, during which they were exposed to uncapped rebar, creating an impalement hazard.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about August 13, 2024, employees performed masonry tasks from a scaffold with spacing between the planks and uprights area greater than 1 inch.  B&B Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1)(i), which was contained in OSHA inspection number 1597299, citation number 1, item number 1, and was affirmed as a final order on March 14, 2023, with respect to a workplace located at 1425 Manhattan Blvd, Harvey La 70058.
