29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: a. On or about August 12, 2024, the employer failed to provide effective guarding to protect employees working at or near the 150-ton OTC hydraulic roll bed press Model: 1866 from struck-by hazards from flying parts ejected while pressing a shaft out of a D4 armature. A press operator was exposed to struck-by hazards when the press tooling slipped out of the press bed and struck the employee in the chest, resulting in eight broken ribs and a perforated liver.
