On or about August 15, 2024, and times prior thereto, in the inertial friction welder area, employees performed maintenance and servicing work on a inertial friction welder by changing the saw blade without energy control procedures being developed or utilized.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about August 19, 2024, and times prior thereto, in the inertial friction welder area, employee was operating a inertial friction welder machine without a functional safety interlock.
