29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about August 20, 2024, employees were exposed to eye injury while operating pneumatic nail guns without the benefit of eye protection.   No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On August 20, 2024, employees performing roofing work were exposed to fall hazards of approximately 23 feet.  Sammy's Roofing Inc., was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1688146, citation number 1, item number 1, and was affirmed as a final order on September 26, 2023, with respect to a workplace located at 754 Greenbriar Lane, Lake Forest, IL 60045.  No abatement certification or documentation is required for this item.

CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:     a)  On August 20, 2024 an extension ladder used by employees to gain access to the working surface did not have its side rails extended at least 3 feet above the surface.   Sammy's Roofing Inc., was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1688146, citation number 1, item number 2, and was affirmed as a final order on September 26, 2023, with respect to a workplace located at 754 Greenbriar Lane, Lake Forest, IL 60045.
