29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about August 20, 2024, at a jobsite located at 6600 Block of W. Sprague RD N. Royalton Ohio 44133; the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth varying from approximately 6.4 to 7 feet. The trench was determined to have type "B" soil.
