29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) 13929 Garfield Dr., Thornton, CO: On or about August 20, 2024, and at times prior, the exposing employer did not ensure that each employee engaged in roofing operations was trained to recognize the hazards of working at elevation and trained in the use of fall protection. Employees installing new shingles on a one-story residential home did not utilize a fall protection system. This condition exposed employees to a fall hazard of approximately 10 feet to the ground.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: (a) 13929 Garfield Dr., Thornton, CO: On or about August 20, 2024, and at times prior, the exposing employer did not ensure that the siderails of a ladder, which was used to access the roof of a one-story residential home, extended at least 3 feet above the landing surface, in that the ladder only extended approximately 6-inches above the landing. This condition exposed employees to a fall hazard of approximately 10 feet to the ground.
